The show consists of hour long episodes throughout a shift at the Pittsburgh Trauma Medical Center (PTMC). The duration of the episodes portrays the reality of a workday in the emergency department. Instead of random time jumps, viewers watch the shift unfold in real time.
The main storyline focuses on Dr. Michael “Robby” Robinavitch, who is the attending of the day shift at the Pitt. Dr. Robby does what he must throughout the day to hold the emergency department together. Along his storyline, the show also follows several other doctors, surgeons, residents, student doctors and patients.
Viewers in healthcare claim that the show is extremely realistic, considering the accurate dialogue and relevant chaos. Throughout the episodes, the high stress events manage to reveal more characteristics of the PTMC staff.
